Add (documents>files) your course evaluations from each semester. Add each course as an individual PDF file and labeled by semester and course number.
You have the chance to name the file whatever you want when you upload (regardless of what the file may be named on your own computer). This allows you to use a naming convention that is easy for the Personnel Committee to decipher. Just make sure the semester, year, and course number are obvious.
Course evaluations are available through Institutional Research and your chair. If there are fewer than 3 students in the course, or fewer than 3 have turned in course evaluations, you will not receive evaluations. It is helpful to the Committee to communicate if any course does not have evaluations and why.
